5．トラウマの層
トラウマケースを扱うことを前提としているのだから、この層について改めて論じるという意味は分かりにくいかもしれない。ただしトラウマの性質や症状の種類が予後を占う意味でも重要であることが最近になり理解されるようになってきている。そしてそこにはPTSDの分類の前進やそれに関連したポリヴェーガル理論の寄与があった。その中でも特筆すべきなのは、PTSDの「解離タイプ」(Lanius, et al, 2010,2012)といわれる新たな分類の提唱である。
Lanius ら( 2010,2012)によれば、PTSDの診断を満たすケースの11～30％は離人体験や非現実体験などの解離を伴うタイプである（Eidhof,
2019）。従来のPTSDの理解においては、トラウマを思い起こさせるような刺激により、患者は脈拍の高進等の交感神経系の緊張や、前頭前野の活動低下、扁桃核の活動高進が起きるとされてきた。しかし解離を伴うサブタイプではそれとは逆の反応が起きることがわかった(Eidhof, 2019)。そしてこのサブタイプに属する患者はより機能の低下が深刻で（Boyd
et al 2018））、人生早期のトラウマもより深刻で、また合併症も多い（Blevins,
et al 2014,Boyd et al 2018。）一般に解離や抑うつを伴うPTSDには暴露療法は向かないとされるが、それには異論もある(Hagenaars, et al, 2010)。
